Coordinated care for people nearing the end of life – feedback wanted

This document will express the perspective of people nearing the end of life.The narrative for coordinated care has been a landmark document in changing the conversation about integration of health and social care and has been adopted by NHS England. There are currently four new branches of the narrative in development, each giving the perspective of a particular group of people. The others are for frail and older people, children and young people with complex needs and people with continuing mental health needs. NHS Networks
